ABSTRACT

Objective: To investigate the application value of diffusion kurtosis imaging (DKI) for evaluating brain parenchyma microstructures changes in hypertensive patients with normal blood pressure control. Methods: Totally 22 patients with primary hypertension with blood pressure controlled in normal range were enrolled in observation group, while 19 healthy volunteers were selected as control group. DKI was performed in all subjects, and the mean kurtosis (MK), kurtosis anisotropy (KA), axial kurtosis (AK) and radial kurtosis (RK) of each brain area were measured and compared between the two groups. Results: MK values in right frontal lobe, right inner capsule knee and right cerebellum of observation group were lower than that of control group, whereas in right parietal lobe was higher than in control group(all P<0.05). The pontine KA value in observation group was lower than that in control group (P<0.05). AK values of left parietal lobe and right pallidum of observation group were lower than that in control group, of left red nucleus, right cerebellum and pontine were higher than that in control group (all P<0.05). The values of RK in right inner capsule knee, right cerebellum and left occipital lobe in observation group were lower than that in control group, and of right putamen was higher than that in control group (all P<0.05). Conclusion: DKI can be used to monitor changes of brain parenchyma microstructures in hypertensive patients with normal blood pressure control.

ABSTRACT

Objecive@#To assess the clinical outcomes of patients who received modified penile augmentation by free dermal-fat grafting.@*Methods@#From April 2012 to December 2014, a total of 15 male adults (18-24 years of age) after hypospadias repairs were included. They underwent modified penile augmentation, including girth enhancement using free dermal-fat grafting, and penile elongation (suprapubic skin advancement-ligamentolysis). Penile measurements were performed using ruler before operation and 6 months after operation. The outcome was evaluated by patients, based on the Male Genital Image Scale. SPSS 22.0 was used to analyze the data. In-paired t-test and Wilcoxon test were applied.@*Results@#In 6-84 months of follow-up, all patients achieved excellent cosmetic results, and satisfied with the appearance and diameter. In weak state, the penile length increased from (5.03±0.47) cm to (6.69±0.49) cm. The increased value was (1.67±0.24) cm (t=8.6, P<0.001). Under traction, the penile length increased from (7.29±0.74) cm to (9.21±0.73) cm. The increased value was (1.93±0.21) cm (t=7.8, P<0.001). In weak state, the diameter of the middle part of penis increased from (7.16±0.25) cm to (8.69±0.44) cm, with the increased value of (1.53±0.23) cm (t=8.2, P<0.001). The MGIS score grew from 31.73±4.86 to 40.20±4.54(Z=3.2, P=0.001).@*Conclusions@#This modified technique is safe and effective in enlarging penile size for patients who had underwent hypospadias repair. It is confirmed that physical dimensional enhancement does contribute to improving their self-estimation of penile size.

ABSTRACT

Objective To study the feasibility and value of magnetic resonance diffusion tensor imaging (DTI) to monitor non?arteritic anterior ischemic optic neuropathy (NAION). Methods Thirty eight NAION patients (56 eyes) were divided into acute period in 17 eyes, progressive period in 16 eyes and chronic period in 23 eyes at the base of onset time. According to matching principle, 56 eyes in 38 normal controls (NCs) were enrolled. All the patients and NCs underwent MR and DTI scan. The raw data were processed by two experienced radiologists, mean diffusivity (MD), axial diffusivities (λ//), radial diffusivities (λ┴), fractional anisotropy (FA) and Length value were got. The independent sample t test was used for the parameter values between the NAION group and the NCs group. A single factor variance analysis was used to compare the parameters among different stages of NAION group. Results Compared to the NCs group, the values of FA and Length in NAION group were reduced [0.20±0.11 vs 0.31±0.12, (5.85±0.92) vs (65.11± 6.89) mm], and the differences were statistically significant (t=-4.28,-5.25;P<0.05). The values of MD andλ┴were increased [(0.16±0.04)×10-3 vs (0.10±0.04)×10-3 mm2/s, (0.16±0.05)×10-3 vs (0.09±0.03)×10-3 mm2/s] in NAION group and the differences were statistically significant (t=6.83, 7.10;P<0.05). The value of FA and Length in acute period, progressive period and chronic period of the NAION group decreased differently compared to the NCs group. At the same time, the value of MD value and λ┴in the three periods of the NAION group increased compared to the NCs groupand the differences were statistically significant (P<0.05). The value of FA between the acute period, the progressive period, and the chronic period of NAION group were statistically signficant (F=10.88, P<0.05). However, no significant differences were found in the values of MD, λ┴and Length of the NAION group (F=0.23, 0.64, 0.33, 1.38;P=0.79, 0.54, 0.72, 0.27). Conclusion The parameters of DTI can be used to monitor the damage of optic nerve and development in NAION.

ABSTRACT

Objective@#To sum up the clinical manifestations and the method for repairing of secondary deformities of postoperation of hypospadias.@*Methods@#Applying balanus tissue flaps and urethral meatus plastic could correct the flat balanus and prevent urethral meatus from falling back. Resecting scar completely in penis and rebuilding urethral tissues could treat secondary curved penis. Applying modified penile augmentation which involved girth enhancement by a free dermal-fat graft and penile elongation (suprapubic skin advancement-ligamentolysis) could correct shorter and much smaller penis.@*Results@#Among January 1st 2007 to December 31st 2016, 62 cases were treated.All of them were male. Their ages were from 14 to 34 years old.55 cases were followed up from 6 months to 9 years after operations, but 7 cases could not be followed up because of the change of addresses and phone numbers.All patients achieved excellent cosmetic result. None of them had complications like urethral fistula.@*Conclusions@#After operations of hypospadias, the secondary deformities may happen when the children grow up.And these deformities always need to be corrected with operations. Balanoplasty, correction of penis and the penile augmentation are good methods for secondary deformities.Consequently, a good penis appearance and function can be acquired.

ABSTRACT

Objective@#To explore the blood supply of expanded lateral neck flap combined with posterior auricular flap pedicled on anterior neck and the feasibility of this kind of flap for facial vulnus′resurfacing.@*Methods@#At the first stage of the treatment, one rectangular expander was implanted in lateral neck in the subcutaneous pockets, overlying the platysma, the volume of the expander is about 200-600 ml. After 2-3 months inflation, the pre-expanded lateral neck flap combined with posterior auricular flap was advanced at the second stage of the operation, the flap was rotated to lateral face to resurface the vulnus defects located on anterior auricle.@*Results@#Twelve cases of facial wounds were included in this research from January 2009 to November 2016. The size of the expanded flaps were ranged from 6 cm×12 cm to 7 cm×15 cm. According to 2 to 24 months follow-up postoperatively, 10 months in average, two flaps showed venous retardation at distal part of expanded flaps, which were recorvered with one-week dress changing after excoriation. The other ten flaps survived with good appearance and function. The flaps matched well to the recipient defects in terms of color, character and elasticity. The appearance and function of face and neck were ideally adjusted after treatment.@*Conclusions@#By expanding lateral neck skin overlying platysma, the expanded lateral neck flap combined with posterior auricular flap pedicled on anterior neck was obtained with high quality and maximum use of blood supply in the anterior neck, which guaranteed appearance and function of the donor area and the recipient area at the same time.

ABSTRACT

Objective@#To investigate the therapeuctic effect of one-stage reconstruction of distal urethra with free graft of tublar oral mucosa.@*Methods@#Two strips of oral mucosa graft( 0.4—0.6 cm in width), were harvested and sutured around an oiled silk roll to form mucosa tube. The mucosa tube was used to reconstruct distal urethra. Postoperative pressure dressing and earlier urination were recommended.@*Results@#From May 2007 to October 2015, 16 cases with distal urethra defect or stenosis were treated with this method. The urethra defect was 2—4 cm in length. Urethral fistula happened in 3 patients. All the other 13 cases healed primarily. 10 cases were followed up for 1—5 years by telephone with normal function.@*Conclusions@#One-stage reconstruction with free graft of bulbar oral mucosa is suitable and reliable for distal urethra defect less than 4 cm in length.

ABSTRACT

<p><b>OBJECTIVE</b>To discuss the methods for urethral external meatus and glanuloplasty after correction of hypospadias.</p><p><b>METHODS</b>The V shape flap on the dorsal side of glan, combined with the bilateral glan flaps were moved to the ventral side. The flap at the ventral side of urethral external meatus was turned over. Then the urethral external meatus was repositioned to the top end of glan with the ventral defects covered by the dorsal flaps. Then the coniform glan was reconstructed.</p><p><b>RESULTS</b>From January 2008 to December 2013, 28 cases were treated, including glandular hypospadias, postoperative retraction of external urethral meatus and meatal stenosis. 21 patients were followed up for 1 -12 months (average, 1 month) with marked improvement of glan appearance and retraction of external meatus. No meatal stenosis happened.</p><p><b>CONCLUSIONS</b>Postoperative retraction of urethral external meatus and meatal stenosis can be corrected by V shape flap on the dorsal side of glan combined with the bilateral glan flaps. The flat appearance of glan can be improved. It is an ideal method for glandular hypospadias.</p>

ABSTRACT

We developed a microfluidic device to integrate sample introduction, bacteria culturing and results reading. The identification of multiple bacteria was achieved by combining the spatial resolution of the arrayed bacteria culture chambers and the color resolution benefited from the bacteria specific chromogenic media. A set of 4 common pathogenic bacteria responsible for urinary tract infection were used as a model to test the microfluidic assay. Our results showed that the bacteria identification assay can be completed in 15 h, with a limit of detection (LOD) of bacteria density down to 10 cfu / mL. Clinical sample testing using the microchip approach showed a coincidence rate of 96. 3% as compared with the conventional method. The developed microfluidic approach is simple and rapid, thus hold the potential to serve as a powerful tool for detection of multiple bacteria.

ABSTRACT

<p><b>OBJECTIVE</b>To compare the effect of three methods for penile reconstruction.</p><p><b>METHODS</b>From Sept. 2000 to Dec. 2012, penile reconstruction was performed in 48 cases with free scapular flap transplantation, or pedicled superficial circumflex iliac artery skin flap, or anterolateral thigh skin flap pedicled with lateral circumflex femoral artery. The flaps were 12.0-14.5 cm in length, 10-12 cm in width and the urethra perimeter was about 2-3 cm.</p><p><b>RESULTS</b>Free scapular flaps were used in 21 cases, with flap failure in 3 cases due to insufficient blood supply. Malleable penile prosthesis was implanted in 11 cases, which was exposed in 3 cases. Groin skin flaps were used in 21 cases, with flap failure in 8 cases due to insufficient blood supply. Malleable penile prostheses were implanted in 7 cases, which was exposed in 3 cases. Anterolateral thigh skin flaps were used in 6 cases, with flap failure in 2 cases due to necrosis and infection. No prosthesis was implanted in this group. All the failure cases underwent secondary reconstruction with successful results. 37 cases were followed up for 1-6 years, avevage 2.5 years. 26 cases were satisfied with the results of appearance and function.</p><p><b>CONCLUSIONS</b>Among these three methods, the free scapular flap is most recommended for its high survival rate, low complication rate and high patient satisfaction.</p>

ABSTRACT

ObjectiveTo evaluate the clinical application of bidirectional Glenn shunt without extracorporeal circulation(ECC) on treatment of children with complicated congenital heart disease (CHD).MethodsForty-six patients with complicated CHD(without ECC group) underwent bidirectional Glenn shunt without ECC,and 40 patients with complicated CHD (with ECC group) underwent bidirectional Glenn shunt with ECC.The therapeutic effect was compared between two groups.ResultsThere was no operative mortality in two groups.The pulmonary artery pressure in without ECC group was significantly lower than that in with ECC group [( 16.7 ± 1.2) mm Hg ( 1 mm Hg =0.133 kPa) vs.( 18.9 ± 1.0) mm Hg,t =4.686,P=0.026 ].Duration of respirator assistance after operation in without ECC group was significantly lower than that in with ECC group [ ( 12.2 ± 2.7) h vs.( 19.2 ± 2.8) h,t =2.972,P =0.041 ].There was no significant difference in saturation of blood oxygen and the rate of chylothorax after operation between two groups (P>0.05).The symptoms and signs including cyanosis and breath were markedly alleviative in all patients.ConclusionBidirectional Glenn shunt without ECC is an effective and safe method for complicated CHD.

ABSTRACT

Objective:To explore the effects and mechanisms of insulin postconditioning on myocardial protection in isolated rat hearts.Methods:32 Wistar rats were randomly divided intocontrol group,ischemia/reperfusion group(I/R group),low dose insulin postconditioning group(L-POST group)and high dose insulin postconditioning group(H-POST group),with 8 rats in each group.Langendorff and neely perfusion models were steadily established,then the rat hearts were arrested by cardioplegia(St.Thomas No.2 solution,STH-2)after perfusion for 30 min.These arrested hearts were remained for 30 min at 4℃and then underwent reperfusion for 60 min.In L-POST group and H-POST group,the arrested hearts were reperfused by Krebs-Henseleit buffer solution(K-H)with insulin.The content changes of myocardial eNOS and PKC were detected by immunohistochemistry and Western blo(tWB),the AI(Apoptosis index,AI)of cardiomyocyte was detected by TUNEL.Results:Compared with control group,higher expression of eNOS and PKC and higher AI were detected in I/R group、L-POST group and H-POST group (P

ABSTRACT

On the basis of the well-established principles and techniques about flow chamber, we have designed and made a kind of parallel plate system to apply steady fluid shear stress to osteoclast-like cells etc. in vitro. Biocompatible rubber and metal clamping apparatus made of aluminium alloy are used to seal the flow chamber without changing its even height designed beforehand. The influence of hydrostatic pressure on cells is minimized by adjusting the glass slide and the fluid surface in the upper reservoir to the same horizontal plane. This system can be used to investigate the responses of osteoclast-like cells etc. to fluid shear force in terms of morphology, physiology or biochemistry.

ABSTRACT

<p><b>OBJECTIVE</b>The purpose of this study was to compare the clinical application and effect of two temporary restorations materials: composite and self-cured acrylic resin.</p><p><b>METHODS</b>153 composite temporary crowns (and bridges) placed in 44 patients and 144 self-cured acrylic resin temporary crowns (and bridges) placed in 49 patients were evaluated according to the California Dental Association's (CDA) criteria and questionnaire.</p><p><b>RESULTS</b>It showed that composite temporary crowns (and bridges) had better surface smoothness, marginal integrity and anatomic form than self-cured acrylic resin temporary crowns (and bridges). In the 49 patients with self-cured acrylic resin temporary crowns (and bridges), 67% and 88% of them complained of food impaction and mouth odor, while only 2% and 30% in the 44 patients with composite temporary crowns (and bridges) had the same complaint.</p><p><b>CONCLUSION</b>Between the two temporary restoration materials, composite is better than self-cured acrylic resin.</p>

ABSTRACT

Objective To establish a reversed -phase HPLC m ethod for the determination of psora len and isopsoralen in Fukangbao capsule.Methods The contents of Psoralen and Isopsoralen were assayed on a ODS -C 18 column with a mobile phase of methanol -water(40∶60)at a column temperature of 35℃.The fl ow rate was 1.0mL /min.The detection wave-length was at 247nm.Results The linear ranges of Psoralen and Iso psoralen were 1.05~10.52?g /mL(r =0.9999)and1.02~10.20?g /mL(r =0.9999)respectively.Their recoveries were within 97.5%(Psoralen)and 100.8%(Isopsoralen),and both of their RSD were 0.6%.Conclusion This method is simple,rapid and accu rate and suitable for quantity -limiting control of Fukan gbao capsule.

ABSTRACT

AIM To study the relation between the antiarrthymic effect and the inhibition of slow reaction cardiomyocyte of ethacizine(EC). METHODS The beating rate and action potentials of cultured slow response cardiomyocyte from neonatal rats was recorded. RESULTS EC 0 1~5 0 ?mol?L -1 produced a concentration dependent suppressing effect on spontaneous beating rate of cultured cardiac cells with IC 50 of 0 17 ?mol?L -1 and the maximal inhibitory rate was 52%.The steady state of inhibition to beating rate was achieved after adding EC for 15~30 min. The effective inhibition of beating rate by EC lasted for over 1 h.As EC 2 5 ?mol?L -1 depressed beating rate to the steady state ,effects of CaCl 2 2 mmol?L -1 and Iso 1 ?mol?L -1 on beating rate were partially reduced,and the effect of aconitine 0 2 mg?L -1 was abolished. EC 0 3~2 5 ?mol?L -1 pruduced the concentration dependent depression on APA,V max ,OS,SP 4 and MDP,and prolongation of SCL of the slow response action potentials.APD 90 was also lengthened at the concentration of EC 0 9~2 5 ?mol?L -1 . CONCLUSIONS The effective depression of EC on abnormal automaticity and conductance of the slow response cardiomyocyte in heart ischemic zone may be an important mechanism for its effective termination on some arrhythimas.EC may depress Ca 2+ ,especially Na + currents on slow response cardiomyocyte.

0 05).③The level of the expression of AT 1a mRNA of the NO group rats was decreased significantly after hypertension of appeared and that of AT 1b mRNA of the LO group rats increased markedly after Losartan was administered. CONCLUSION Losartan has antihypertensive effects and can reverse myocardial hypertrophy induced by renovascular hypertension. Losartan regulates the mRNAs of the AT 1 receptor subtypes differently, which suggests it possibly has selectivity for AT 1a or AT 1b .

ABSTRACT

The effects of ranitidine (Ran) on cultured myocardial cells of neonatal rats were observed. Ran 100 ?mol?L-1 took no effect on spontaneous beating rates of cultured myocardial cells, but it decreased the beating rates increased by histamine 10 ?mol?L-1 and did not inhibit the beating rates increased by isoprenaline 2?mol?L-1. Histamine 10?mol?L-1 increased the amplitude, maximal rate of rise and pro-longed action potential duration at 50% (APD50) and 90% (APD90) repolarization. The effect was abolished by Ran 100?mol?L-1. The results suggested that Ran may possess antiarrhythmic action induced by histamine's increase.
